This is a required step for launching your new Integrity website. If your office is open and accepting patients we require office photos prior to constructing your Integrity website. If your office is not yet open we will set your site up with stock photos for thirty (30) days; your office photos must be received by the end of this 30 day window.

For more information on Integrity’s photo upload and editing policy please see below or visit the FAQ page. Uploading of photos constitutes acceptance of the upload and editing policy. No retractions or changes are permitted once photos have been uploaded, even if the photos do not meet the upload guidelines. Offices may either replace photos themselves or request a premium staff update ($75) to have Integrity staff place new photos if unhappy with the appearance of the uploaded photos on the website.
This form requires Firefox 31 or higher, Chrome 34 or higher, or Internet Explorer 11 or higher, running on Windows 7/8/8.1/10 or on Mac OS X 10.8 or later. If your browser does not meet these requirements, please submit photos from a browser that does meet the requirements. The form may not work and Integrity may not receive your photos if there is a browser error.
